How to download and Print NSS monthly evaluation Form.
National Service personnels who find themselves serving in the Government institutions or offices are usually mandated to download, print, and have their monthly evaluation form signed by their supervisors at their respective agencies. The signed forms are then submitted to their allocated district National Service (NSS) office to be processed.
Why is submitting the form important?
The monthly allowance of National Service Personnels will not be processed if the monthly evaluation forms are not submitted. NSS personnels are tasked to do so to verify how active they were at work throughout the month.
Steps to Download National Service Monthly Evaluation Form from the NSS Dashboard
Below are to steps that will guide you into downloading your monthly evaluation form from the NSS dashboard:
- Visit your NSS Dashboard here – https://portal.nss.gov.gh/sign-in
- Log in with your Email and Password as recognized by NSS.
- After successfully logging into the NSS dashboard, navigate to download the Monthly Evaluation Form (for the month you want to submit).
- You can now save the NSS Monthly evaluation form on your PC or mobile phone or proceed to download instantly.
Note: Before saving the evaluation form or printing from the dashboard, make sure your uploaded photo, NSS Ezwich number, Month and Year, and QR code are all present on the form.
How (steps) to print and Submit National Service Monthly Evaluation Form
- Send the monthly evaluation form copied from the dashboard to any printing shop to print it.
- After printing, write your district number above the QR Code and sign at the section provided for National Service Personnel.
- Present the form to the Supervisor you work with to sign (or endorse your form with the agency’s stamp if possible)
- Make photocopies of your Monthly evaluation form (it is recommended you make at least 2 copies) and submit it to your district or municipal National Service Office.
- Check your dashboard again few days after you have submitted the forms to see if your evaluation was successfully submitted.
NB: Right after the monthly evaluation form is submitted, processing of the allowance begins, therefore it is highly recommended to always submit the form on time.
